A Step-by-step Guide
1. Register as a patient with the Department of Health
Visit the Department of Health Medical Marijuana Program login page and select "Adult Patient Registration". You will need your PA driver’s license or state identification in order to complete this. Once you’ve registered, you will receive a unique Patient ID number. Take this ID number down; your certifying practitioner will need it during your certification consultation. 2. See an approved practitioner to get certified
Now that you’ve registered with the DOH, you’ll need to undergo a medical certification. Sign up for a certification consultation with one of Herbal Care Rx’s certifying practitioners. After filling out our patient intake questionnaire and booking an appointment, you will have your telehealth appointment over the phone with one of our certifying practitioners.
3. Log in to your PA Department of Health Account
After a certification has been submitted, log back in to your Department of Health account and make the $50 payment for their card fee. If you qualify for a fee waiver, select which program you qualify for and submit the form. When you’ve completed this step, the Department of Health will begin the process of printing and mailing your medical card to you. Your card will be valid for one year.
Once you receive your card, it is good for use at any PA dispensary. Bring it there and talk to your local dispensary’s pharmacist about a treatment plan that is right for you.
After your registration has been accepted
You will be prompted to create a username and password. It is important to save this information, as you will need to log into your account to make payments to the Department of Health and to check the status of your card.
When you have created your account, you will also receive a 6-digit Patient ID number. Write this number down. You should now be ready for your appointment!
